Resthaven is proud to announce the launch of the Know Your Score tool for residents of its Independent Living communities—becoming the first senior living organization in West Michigan to offer this innovative, evidence-based program at no cost to residents.
Developed by researchers at the University of Michigan School of Nursing, Know Your Score is part of the nationally recognized HealthyLifetime® initiative. This evidence-based self-assessment tool helps older adults evaluate their risk of losing independence and offers actionable insights to support healthy aging and overall well-being.

A New Approach to Independence
The Know Your Score assessment takes just 15 minutes to complete and generates a personalized report highlighting both strengths and potential areas for improvement. Unlike traditional health screenings, the tool examines the often-overlooked factors that influence long-term independence, such as social connection, problem-solving and coping skills, mental health and emotional well-being, sleep, nutrition, and daily routines.
These hidden drivers can quietly erode independence over time. By recognizing risk factors early, residents can take proactive steps to maintain their autonomy, confidence, and quality of life.
Studies have shown the HealthyLifetime® model can extend years of independent living, enhance overall quality of life, reduce healthcare costs by up to 50%, and increase personal confidence and daily functioning.
